Purchase with HSA or FSA Funds
In the United States, Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) let you use pre-tax dollars to pay for qualified healthcare expenses.
The Ezybandz GuardianOne medical alert device (PERS) is HSA/FSA eligible. Replacement bands are also eligible when purchased as necessary parts for continued use of the device.
Because eligibility rules differ by plan or employer, please confirm with your HSA/FSA provider before purchasing.

Ways to Use Your HSA/FSA Funds

Use an HSA or FSA card at checkout – just like a debit/credit card (MCC 8099 – Medical Services)

Pay out of pocket with any payment method and submit for reimbursement to your provider

Some authorized retailers may accept HSA/FSA cards check with them first

Gather Your Documentation

Itemized receipt or invoice showing:

Product description & SKU (e.g., “GuardianOne Medical Alert Device,” “GuardianOne Replacement Band”)

Price, taxes, and shipping

If your provider requires it, obtain a Letter of Medical Necessity from your healthcare professional.

Submit Your Claim

Log into your HSA/FSA provider’s portal

Complete their claim form and upload your receipt/invoice

For FSAs, confirm purchases fall within your plan year (or its grace period)
Order & Payment Issues

If your HSA/FSA card is declined, confirm your available balance or eligibility with your provider.

You can complete your purchase using another payment method and submit for reimbursement.

Most plans do not allow split payments between HSA/FSA and another card.
Gifting
Federal regulations limit HSA/FSA funds to purchases for the account holder or their eligible dependents. Items bought as gifts for others are not reimbursable.
Returns, Exchanges & Cancellations

Refunds for HSA/FSA purchases must be issued back to the original card (open, active accounts only).

Orders may be cancelled before shipment. After shipping, follow our standard return policy.

Refunds must go back to the same HSA/FSA account used for the purchase.
